はじめに

産業医の役割は、制度や職場のあり方と切り離して考えることはできません。

過去の運用には、その時代に必要とされた背景があります。

当時の制度や職場の管理体制のなかで、産業医が安全を守るために担ってきた役割もあります。

そして、制度が変わり、職場の管理機能が変われば、産業医の役割の境界もまた、現在の状況に合わせて更新されていきます。

過去の意見には、その時代の背景がある

かつて、長時間労働による健康障害が大きな社会問題となり、労働時間管理の仕組みも現在ほど整っていなかった時代がありました。

そのような環境のなかでは、産業医が勤務時間について具体的な意見を示すことが、職場の安全を支える役割を担う場面もありました。

産業医の意見は、本人の健康状態だけでなく、その時点の制度や、職場が備えている管理機能との関係のなかで形づくられます。

過重な勤務を把握し、必要な対応につなげること。

これらは、職場が組織として担う管理機能です。

一方、産業医は、一人ひとりの健康状態と業務との関係を見て、安全に働くために必要な条件を示します。

同じ勤務時間に関わることであっても、職場が行う労働時間管理と、産業医が示す健康上の意見とでは、その役割が異なります。

制度が担うこと。

職場が管理すること。

産業医が健康上の観点から判断すること。

制度や職場の管理機能が変化するなかで、その間にある役割の境界も、現在の職場に合わせて整理されていきます。

産業医が示すのは、健康上必要な条件

産業医が示すのは、本人の健康状態と業務との関係から、安全に働くために必要となる条件です。

たとえば、勤務と勤務の間に必要な休息が確保されること。

急変のおそれがある場合に、速やかに周囲の支援を得られること。

その条件を、勤務時間や業務内容、人員配置などにどのように反映するかは、職場が具体的に考えます。

健康上必要な条件を具体化した結果として、勤務時間について明確な意見を示す必要が生じることもあります。

大切なのは、時間数を示すことそのものではなく、その条件が、本人の健康状態と業務との接点から導かれていることです。

おわりに

産業医の役割は、制度や職場のあり方との関係のなかで形づくられます。

制度が変わり、職場の管理機能が変われば、その境界もまた見直されていきます。

産業医は、本人の健康状態と業務との関係から、健康上必要な条件を示す。

職場は、その条件をもとに、具体的な働き方を考える。

何を産業医が判断し、何を職場の判断に委ねるのか。

その境界を、現在の職場に合わせて更新し続けること。

それもまた、産業医の専門性です。

The role of an occupational physician cannot be considered separately from the systems and workplace structures in which it is performed.

Past occupational health opinions reflected the circumstances of their time. When systems for managing working hours were less developed, occupational physicians sometimes played a more direct role in specifying working-hour conditions to support workplace safety.

As workplace management functions evolve, the boundaries of the occupational physician’s role should also be reconsidered. The workplace manages working hours, while the occupational physician identifies the health-related conditions necessary for safe work.

These conditions may include ensuring sufficient rest between work periods or access to prompt assistance in the event of an acute health problem. The workplace then determines how to reflect those conditions in working hours, job duties, staffing, and other practical arrangements.

When health-related conditions are translated into practice, explicit working-hour conditions may sometimes be necessary. What matters is not the act of specifying a particular number of hours, but whether the condition is derived from the relationship between the employee’s health and the actual demands of the job.

Continually reviewing what should be determined by occupational health professionals and what should remain within workplace management is itself part of occupational health professionalism.
